| Requirements and Responsibilities For Case Manager (LPN) |
| Coordinates the overall care plan for patients under the direct supervision of a registered nurse (RN) or physician. Evaluates the needs of the patient, the resources available, and recommends the plan for the best outcome. Collaborates with physicians and other medical staff members to facilitate the progression of care. Monitors the patient's progress and acts as a liaison between the patient, medical staff, and insurers. Develops a discharge plan that provides the best available resources to meet ongoing patient needs. Identifies patient care issues and makes recommendations. Requires a high school diploma or equivalent. Requires graduation from approved LPN educational program. May require a state license. Typically reports to a registered nurse (rn) or supervisor. Years of experience may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. |
